Is it permissible to say all Takbirs, the Samiallah, the Bihawlillah, and the Taslim aloud during Zuhr and Asr prayers? Additionally, can we recite the Bismillah aloud before Surah Fatiha and other Surahs in these prayers?

Saying all the Takbir and other sayings in Zuhr and Asr aloud is permissible. Reciting the AlHamd and the other Sura should not be aloud in Zuhr and Asr. Reciting Bismillah Arrahman Arraheem aloud is recommended in Zuhr and Asr, while it is obligatory in Maghrib, Isha and Farr prayers.

An excessive doubter believes that he has skipped Surah Al-Fatiha and recites it after the second Surah. Is his prayer in order?

As he did not leave reciting Sura Al-Hamd deliberately, his Prayer is valid. Reciting Al-Fatiha after the second Sura is allowed and it does not harm his Prayer.

If your mistake in the reciting of Sura Al-Fatimah was unintentional and due to un avoidable ignorance, you don't need to repeat you Prayers.

Is it obligatory to recite a different Surah along with Surah Al-Fatiha in the first or second Rakat instead of only Surah Al-Ikhlas?

Reciting Sura Al-Fatiha is obligatory and compulsory in first and second Rak’ats. After Al-Fatiha, you must recite another Sura whether Sura Ikhlas or any other Sura. Reciting Sura Ikhlas or Sura Al-Qadr or Al-Kafiroon after Al-Fatiha in Salah has been mentioned in many Hadeeths.
