It is allowed to kill insects if there is a possibility of harm from it on you, your family and others. This is allowed in every month.

State of Ehram ( in Hajj and Umra) has special rules.

I have not fasted for last 6 years of Ramadan and I was not sick or in any situation that exempted me. Where do I start from and how do I go about making up what is due on me?

Simply, start after the Month of Ramadhan making up (Qadha) of all the days which you missed and you must also pay Kaffara of ever day you deliberately missed by feeding 60 poor believers for every day of missed fast.

Niyyat is nothing more than the intention in your mind or heart to fast for the sake of Allah (SWT). Just having this intention to fast the whole of the month of Ramadan is enough for the whole month.

there is no problem if husband and wife have physical relationship during the night between iftar and suhur.
