If a male accidentally ejaculates due to masturbation while fasting in Ramadan without intending to, does he need to perform both Qadha and Kaffarah, or is it only required if the ejaculation was intentional?

If the ejaculation occurred without intending to do that, Qadha is must of that fast. Kaffara is obligatory if it was done intentionally. But how can one masturbate with out intending to do it? Was he sleeping. If he masturbates while he was awake then he has committed it intentionally. If he masturbated but his intention was not to reach to ejaculation, and ejaculation occurred with out his control, that might be possible.

If I missed fasting during the entire Ramadan last year and could only fast for a few days before the next Ramadan, will the missed fasts carry over to the following year?

Yes. All missed fasts which you did not re do as Qadha, must be kept as soon as you can.

Can couples who are both exempt from fasting due to medical reasons engage in intimacy during the days of Ramadan?

If both of them were really exempted from fasting due to illness which prevents fasting or due to traveling, they will then be allowed to engage in intimacy during the day of month of Ramadan.

If my 9-year-old sister ate during the day in Ramadan due to feeling hungry, what is the ruling regarding making up for that broken fast? Is it necessary for her to repeat it or should she observe two consecutive months of fasting instead?

The age of taklif for a female is completion of nine lunar years, which in Gregorian calendar is 8 years, 8 months and 20 days (approximately). 

If she is younger than this, she does not need to make up to anything, and her fasting is just for learning. If she is of taklif age, and she was extremely hungry, unable to continue fasting, she just makes up this day later on as qadha`, and there is no kaffarah. 

It is always strongly recommended to wake up for suhur (pre-dawn) time and have a healthy meal, drink water, etc., to be able to avoid severe hunger during the day, especially for younger people.
