During prayer, should i say ash hadu AN la ilaha il Allah or ash hadu AL la ilaha il Allah?

Is it permissible to directly address Allah (swt) in the Tashahhud during my prayers in this way: “الحمد لله أشهد أن لا إله إلا أنت وحدك لا شريك لك وأشهد أن محمدا عبدك ورسولك”?

No. We must pray as the Prophet Muhammad (SAWA) taught us to pray (Pray as you saw me orating صَلّوا كَما رأيتموني أُصَلّي

He was saying Ashhadu ANLA Ilaaha Illallah WAHDAHU Laa Shareeka LAH Wa Ashhadu Anna MUHAMMADAN Abdoho Wa Rasooloh, ALLAHUMMA SALLI Ala Muhammadin Wa AALI Muhammad)

While praying, I was in my second Ruku and leaned forward to rise, but then recalled I needed to perform Tashahud, so I went back down to do it before continuing my prayer. Is my prayer still considered valid?

Yes, your prayer is valid but you need to perform two Sajda of Sahw immediately after your prayer. Sajda of Sahw is prostration and saying : BISMILLAH WA BILLAH ALLAHUMMA SALLI Ala Mohammad Wa AALI Mohammed. Then sitting and prostrating again and repeating the same. Then  saying Tashahhud and Salam.

If someone is unsure whether they have completed four or five Rak'ahs, should they sit, recite the Tashahhud and Salam, then perform one Rak'ah of Salat al-Ihtiyat while standing, or two Rak'ahs while sitting?

In case of the doubt between four of five Rak'ats,nyou should take it as four and recite Tashahhud and Salam, then perform two Sajda of Sahw. There no Salat al-Ehtiyat in this case.
