If there is no barrier between the man and woman, the woman (even if she is mahram) must stand behind the man, such that her place of sajdah be behind where the knees of the man would be when he does sujud.
Saying all the Takbir and other sayings in Zuhr and Asr aloud is permissible. Reciting the AlHamd and the other Sura should not be aloud in Zuhr and Asr. Reciting Bismillah Arrahman Arraheem aloud is recommended in Zuhr and Asr, while it is obligatory in Maghrib, Isha and Farr prayers.
It is Not necessary to perform Sajda Sahw for just not following Tajweed rules in Salah. You should recite as much as you can as in Qira'ah and as in Tajweed.
