During my Iddah period, my husband and I engaged in intimate touching but did not have intercourse. Does this imply that we are married or divorced?

You are married.
If your husband touched you during your Iddah period, the Talaq is nul and void immediately and you are married to one another as you were before the divorce.

I had only completed the Nikah and am now getting a divorce after having sexual relations with contraception. Do I still need to observe the Iddah period?

Yes. Iddah of Talaq is must even if the sexual relation was with contraception. Your Iddah after Talaq will be three menstrual periods.

There is no 'iddah for the wife to observe while "filing" for divorce. She must observe 'iddah only after the Islamic divorce is recited. When the talaq is recited, her 'iddah period will be three menstrual cycles.

If a husband and wife have been living separately for more than 6 months, and the wife requests Khula, is the Iddah period mandatory for the wife? If so, what is the correct way to observe it?

Yes, the 'iddah period must still be observed by the wife, even though a long time has passed from their separation, as long as she is not ya`isah. 

It is three menstrual cycles, and she does not necessarily need to do anything. The only thing she cannot do at all is get remarried during this time.
