Kaffara is a penalty on those who deliberately break obligatory fast or do not keep it. For those who deliberately don’t fast during the month of Ramadan, for one day,v the Kaffara is either fasting two sequent months or feeding sixty poor believers. It is good to do it as soon as possible and should not be postponed to an extended that it might be forgotten or ignored. Feeding sixty poor believers must be by giving them ready food or food grain and not money. Every ooor believers must must be given either a meal or 750 grams of food grains.

Kaffara is obligatory for every day of deliberate breaking fast beside Qadha of that fast. If you don't have money to feed sixty poor believers, you can fast two months as Kaffara of not fasting deliberately for each day. If you can not do that, you should wait till you are able to earn and feed the poor believers.

It is not kaffarah that they pay. It is fidyah, which is 750g of certain food items, per day. You can give it to a relative, if they are needy, and it can be given at any time.

No, there is no kaffarah or blood-money, etc., as long as it was consensual as well, from her side, and not to hurt her.
