Temporary marriage is conducted in the modern era just like permanent marriage is conducted with slight differences between the temporary and permanent marriage. Both types of marriage need agreement between both man and woman, woman should not be presently married or during Iddah period. Mahr (Dowry) is must from the man to the woman. Permanent marriage end on valid divorce or death, while temporary marriage ends in the end of the agreed period of it or by leaving the remaining period by the husband.

'In temporary marriage, both man and woman agree on a period of the marriage as well as the Mahr of the marriage. She tell him: Zawwajtoka Nafsi ALAL Mahril Ma'loom lilmuddatil Ma'loomah. He replies her: Qabiltu Al-Tazweej LINAFSI lilmuddatil Ma'loomah ALAL Mahril Ma;loom.

From a shar'i perspective, you do not need consent from your father, but I would advice you against engaging in mut'ah, and focus on recovering from the previous relationship, and hopefully finding new marriage. 

As a society, one who engages in such things becomes less wanted for marriage, and our Islamic values require that we stay chaste and modest, aiming at building a family. 

Also, although shar'i wise, you do not need your father's consent, but ethically, morally and socially, involving family in these things is definitely important.

This Muslim woman should not be living with a non-Mahram person anyway. So, she needs to remove herself from that situation. 

As for the man doing mut'ah with her, that's going to be between them, and if they both meet the valid /halal requirements for it.

Yes if the girl is virgin which means that she was never married.
