Numerology should not lead to claim knowing the unseen, nor put allegation on others, nor claiming tributes with out real evidence.

Is it a requirement to use numerology and Ilmul Adad to name a baby according to the beliefs of many Shia elders?

No, it is not required. We do not have any authentic evidence from the Prophet (SAWA) and Ahlul Bayt (AS) to suggest such requirement. It is recommended to name our newly born after the names of Ahlul Bayt and names recommended by them with out going through numerology or Ilmul A'dad. Some elders in the subcontinent think it required but it is just a cultural matter not a religious requirement.

Is it permissible in Islam to follow numerology, which suggests changing the spelling of one's name for the purpose of attracting good fortune?

Good fortune does not come from spelling of our names, but from piousness in our heart and noble deeds. There are many persons with very good names but very bad deeds and vice versa.
