Very often restaurants in the west fry their vegetarian products in the same oil as non-halal meat products. Is it okay to eat? If we don't know if this takes place, are we required to ask at each restaurant?

If one knows that a product is fried with non-halal meat, then it would become najis (impure) and haram to eat. If one didn't know then there is no sin upon them.
