Marriage, also called matrimony or wedlock, is a culturally and often legally recognised union between people called spouses. It establishes rights and obligations between them, as well as between them and their children (if any), and between them and their in-laws. It is nearly a cultural universal, but the definition of marriage varies between cultures and religions, and over time.

To my knowledge, there are no hadith (addressed to either men or women) which discuss choosing between marriage and career (or marriage and studies), although there were both men and women with skilled professions among the sahabah (according to the professions of their society such as healing or craftsmanship). Perhaps it wasn't seen as something that one chooses between. That is, it was assumed that most people would marry and some people would also do other things. 

Although people have had professions and vocations for a long time, it seems that the modern idea of career began a hundred or two years ago, after the Industrial Revolution. 

Similarly, the idea that large numbers of people should invest a large portion of one's life in full-time studies in an institution, while doing little else, is also quite recent. So, the circumstances we have today are different from those in the time of the hadith.

If your father is deceased as well as your paternal grand father, you don't need to obtain permission from any one for your marriage. No doubt, you must keep respecting and serving your mother, but her permission is not compulsory in your marriage. Try to explain your situation to her with high respect to her, but the rule of Islam is above every one.

If the procedure of the smear test does not involve sinful act like exposing your body to non Mahram man, or your private parts to anyone even females, then it is allowed to do it when your surgery suggest that.

We have narrations encouraging marrying from other families and not very near relatives especially when is an inherited illness in the family.

Cousin marriages took place among Ahlul Bayt (AS) like the marriage of Imam Ali (AS) and Lady Fatima (AS) , marriage of Imam Zainul Abideen (AS) and Fatima daughter of Imam Hasan (AS) and many other examples.

Fostering a child is highly recommended in Islam. Fostering a needy child or a needy orphan elevates the foster to high degrees in the Paradise as the Prophet  Muhammad (SAWA) has reportedly said: Myself and a foster of an orphan are together in the Paradise like two fingers (the index finger and the middle).

Fostering person looks after the livelihood needs of the child which he/she wants to foster as far as he/she wants. In case for any reason, the person wants to stop his fostering, he can  stop it and there is no legal obligation on him as the whole fostering is a voluntary noble act. Morally, the foster should do his best to look after his fostered child or look for a person who should look after the child in case he is unable to continue.

Muslim married man is not allowed to do Mut'ah marriage with any woman from People of Book (Christians , Joes and Magi) with out permission from his Muslim wife.

You should try your best to travel to be with your wife at least once every four months, as it is not permissible to leave your young wife more that four months.

A human that she likes (from any time period in history!), probably male version of hur al ayn, maybe hanging around by herself and not bothering with marriage. There are no limitations in jannah :)

You need to keep in touch with her and treat her nicely being a practicing Muslim so that she will see the great manners of Islam. You should try to invite her to Islam as and when you find a suitable chance. Her being non Muslim does not mean always that she denied Islam deliberately. She might had been a victim of ignorance or misleading propaganda.

You should do your best to give the practical evidence that Islam is the best religion.

It is good to pay attention to your intuition.  If something "does not seem right" and feels like a "sacrifice", this could point to a deeper concern about the situation, beyond merely whether or not you will be working outside the home. (Although it could include the dynamics behind why he doesn't want you to work, and his reaction to your situation with your family.)

Anyway, you could give it some thought and see whether or not there is anything else that is bothering you. 

You could also imagine yourself 20 years in the future and ask yourself if you would feel disappointed or resentful if you gave up your career for marriage. You could also ask yourself if you would feel disappointed and like you really lost out if you did not marry this person. 

Also, practically, in case you do get married and the marriage doesn't work out (it happens!) or, God forbid, something happens to your husband, do you have financial resources or a support structure to fall back on (such as living with family), or would it put you/your family in a vulnerable situation? This is also something you could take into consideration when considering whether or not to give up your career.

Beyond that, it is your choice whether or not to marry him, and we can't advise on that here. It is natural for marriage to have compromises and usually people will have to compromise something in a marriage. The question is whether it is a compromise that people are willing to make, or not. So what compromises you and he are willing to make is up to you and him.

As a general note: Life can change a lot, people might have one idea of how they will live but eventually need to make some changes. For instance, sometimes a person might not have a career before a marriage but get one later in life, or someone might leave a career sometime in life. This isn't really important to your situation right now, but is good for people to keep in mind overall.

Yes it is permissible for a woman to propose marriage to a suitable man. She can convey her proposal through a trusted person or through any suitable way like a letter or an email etc. Such proposal should be studied very carefully because it is not common among men, and it might be misunderstood or misused if the man is not pious enough. The man should be with high degree of faith, religious practice and manners.
